Qualifications

Master's Degree in Social Work from CSWE Accredited School.
Oregon Clinical Social Worker License upon hire or Washington Clinical Independent Social Worker License upon hire.
License in states wherever care is delivered.
2 years relevant Social Work experience.

Responsibilities

Independently provides biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy.
Collaborates with the patient and support system to develop evidence-informed care plans with IDT.
Adheres to NASW ethics and SOP while reducing health disparities.
