Job summary

Providence is seeking a Licensed Clinical Social Worker (LCSW) to provide independent biopsychosocial assessments and evidence-informed interventions in Oregon. You will collaborate with patients and IDT to develop comprehensive care plans, ensuring patient self-determination and reducing disparities in care.

The role requires CSWE-accredited MSW and Oregon or Washington LCSW licensure, with additional licenses in states where care is delivered.

Qualifications

• Master's Degree in Social Work from a CSWE Accredited School of Social Work or candidacy status by CSWE.
• Oregon Clinical Social Worker License upon hire.
• Washington Clinical Independent Social Worker License upon hire.
• License in states where care is delivered.

Responsibilities

• Conduct comprehensive biopsychosocial assessments, diagnoses and/or treatments.
• Collaborate with patients and support systems to develop individualized care plans.
• Work with the Inter/Multi-Disciplinary Team (IDT) to implement evidence-informed, person-centered care across the care continuum.
