Job summary

Providence Health & Services in Oregon is seeking an experienced Licensed Clinical Social Worker (LCSW) to perform comprehensive biopsychosocial and spiritual assessments, diagnoses, and implement person-centered care plans within an interdisciplinary team.

You will collaborate with patients and families to develop care plans, address health disparities, and advocate for patients while upholding NASW ethics across the care continuum.

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• Oregon Clinical Social Worker License upon hire (or equivalent Washington license).
• 2 years of relevant Social Work experience; licensure in states where care is delivered.

Responsibilities

• Perform biopsychosocial/spiritual assessments, diagnoses and treatment plans.
• Develop and implement evidence-informed, person-centered care plans across the care continuum.
• Collaborate with the Inter/Multi-Disciplinary Team (IDT) to coordinate care.
• Advocate for patients while adhering to NASW ethics and SOP.
