Job summary

Mercyhealth is seeking a Social Worker to provide counseling, information, and direction to obtain community resources for oncology patients and their families, collaborating with physicians and the care team.

Schedule is 32 hours/week, full time, day shift. Licensure in Illinois or registry eligibility, and BSW degree are required; CPR/BLS within 90 days. The role involves education, referrals, and emotional support within a multidisciplinary setting.

Qualifications

• BSW from an approved graduate school of social work.
• One year of social work experience in a healthcare setting preferred.
• Experience and ability to facilitate groups.

Responsibilities

• Assess patients for continuing care needs and create a plan for providing continuing care with the care team.
• Refer patients or families to community resources and assist with advance directives.
• Educate patients about resources and document activities promptly.
• Provide emotional support and help patients/families understand and accept their condition and medical recommendations; arrange changes in levels of care as needed.
• May be required to work flexible hours.
