Job summary

Mercyhealth is seeking a Social Worker to provide counseling, resources, and coordination for oncology patients and families. The role collaborates with physicians, nurses, and the health team to plan continuing care and facilitate access to community resources.

This full-time position offers 32 hours/week with a day shift. The BSW is required, with preference for 1 year of healthcare social work experience and group facilitation abilities. CPR/BLS certification within 90 days is expected.

Qualifications

• Bachelor of Social Work (BSW) from an approved graduate school of social work.
• One year social work experience in a healthcare setting preferred.
• Experience and ability to facilitate groups.

Responsibilities

• Assess patients for continuing care needs, create plan for continuing care, and confer with physician and team members as needed.
• Refer patient or patient's family to other community resources as indicated; assist patients and families to complete advance directives.
• Educate patients about resources, options and document all activities in a timely manner.
• Provide emotional support, help patients and families understand and accept their condition and medical recommendations; utilize community resources to assist patients to live in the community; arrange changes in levels of care and settings.
• May be required to work flexible hours.
