Mercyhealth is seeking a Social Worker for the Cancer Center in Freeport, IL. The role provides counseling, information, and direction to obtain community resources for oncology patients and families.

The position is 32 hours per week, with day shifts and potential flexible hours. The role emphasizes collaboration with physicians, nurses, and the care team, along with documenting activities and ensuring patient education.

Qualifications

• BSW from an approved graduate school of social work.
• One year social work experience in healthcare setting preferred.
• Licensed to practice in Illinois or registry eligible.
• CPR or BLS certification required within 90 days.

Responsabilités

• Assess patients for continuing care needs and create care plans.
• Refer patients/families to community resources as indicated.
• Educate patients about resources and documents activities for timely record-keeping.
• Provide emotional support and help patients/families understand medical recommendations.
