Beth Israel Lahey Health seeks a compassionate Medical Social Worker to join our integrated care teams in providing psychosocial assessments, care planning, counseling, and resource coordination across inpatient, outpatient, and behavioral health settings.

Responsibilities include crisis intervention, discharge planning, and advocacy for patient rights while collaborating with multidisciplinary teams to ensure equitable access and quality outcomes.

Competenze

• Master’s Degree in Social Work (MSW) from an accredited program.
• Current Massachusetts social work license required; LCSW or LICSW depending on assignment.
• Experience in healthcare, behavioral health, or acute care settings preferred.

Mansioni

• Conduct comprehensive psychosocial assessments to identify patient, family, and environmental needs.
• Develop and monitor individualized care plans, provide counseling, crisis intervention, and emotional support.
• Coordinate services across care continuum and facilitate discharge planning and transitions.
• Advocate for patient rights and help navigate resources, including insurance and community services.
• Collaborate with physicians, nurses, and other professionals to support patient-centered care.
