Job summary

Responsibilities

• Plan, implement, coordinate, evaluate and promote continuity of care.
• Assess health status by interviewing patient and medical history.
• Administer therapies and medications under physician supervision.
• Maintain patient confidentiality and accurate documentation.
• Triages patient phone calls and coordinates care with interdisciplinary team.
