- Assess patient physical, psychological, social, spiritual, educational, developmental, and discharge planning needs.
- Formulate and implement a plan of care in collaboration with the patient/family and health team input.
- Implements the plan of care through direct patient care, coordination, delegation and supervision of the health care team.
