• Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs.
• Formulates and revises a plan of care in collaboration with the patient/family and health team.
• Implements and coordinates plan of care, including supervision of the nursing staff as needed.
• Documents assessment, planning, implementation and evaluation in the patient record.
• Contributes to ongoing quality improvement and staff development.
