Responsibilities

• Build and maintain positive relationships with referring providers to support coordinated patient care.
• Conduct regular outreach through visits, calls, emails, and follow-ups.
• Utilize Salesforce and analytics to assess referral patterns and plan strategies.
• Document outreach activities in Salesforce for transparency and performance tracking.
• Collaborate with leadership and internal teams to align with strategic goals.
• Support educational events and market insights collection.
