Job summary

Cleveland Clinic is seeking a Pharmacy Clinical Specialist for BMT/Leukemia to support oncology patients across inpatient and outpatient settings. You will participate in rounds, verify complex regimens, manage discharge/consult services, and collaborate with multiple pharmacy teams to ensure safe transitions of care.

The role offers opportunities to precept learners, conduct research, and contribute to a growing oncology program.

Qualifications

• Pharmacist licensure and residency/certification requirements as listed.
• Eligibility for state reciprocity within 90 days of hire where applicable.
• PGY2 or equivalent advanced residency experience or significant clinical specialist background.
• Immunization administration training or certification within 90 days.
• ACLS/PALS certifications as applicable.

Responsibilities

• Coordinate and provide pharmaceutical care for patients across inpatient and outpatient settings.
• Participate in daily interdisciplinary rounds and patient care planning.
• Perform pharmacotherapeutic monitoring and provide pharmaceutical care.
• Provide general and patient-specific drug information and education.
• Educate clinical pharmacists, technicians, students, and nurses in pharmacotherapy.

Education

PharmD or Bachelor of Science in Pharmacy from an accredited college of pharmacy

Current state licensure as a Registered Pharmacist (RPh) OR eligible for reciprocation in the current state within 90 days of hire

Post-graduate year 2 residency (PGY2) or fellowship OR PGY-1 plus two years of clinical specialist experience OR four years of clinical specialist experience

Immunization administration certification: BLS through AHA/Red Cross and immunization course or certification within 90 days of hire

ACLS or PALS certification through AHA/Red Cross or within 1 year of hire
