Job summary

Cleveland Clinic Weston Hospital is seeking a Clinical Pharmacist to support the inpatient Hematology/Oncology service in a 256-bed facility. You will participate in multidisciplinary rounds, review and optimize chemotherapy orders in Epic, and ensure medication safety and efficacy for a high-acuity patient population.

The role includes clinical monitoring, medication management, and education of learners, with a fixed schedule from 8:00 am to 4:30 pm.

Qualifications

• PharmD or Bachelor’s of Science in Pharmacy from an accredited College of Pharmacy.
• Current state licensure as a Registered Pharmacist (RPh) or eligible within 90 days of hire.
• Immunization certification or eligibility (BLS/ACLS/PALS) within required timelines.

Responsibilities

• Provide safe, efficient, patient-centered and evidence-based pharmacy services.
• Verify and dispense medications accurately per regulations.
• Monitor medication efficacy and safety; perform therapeutic interventions.
• Document services accurately and participate in medication safety initiatives.
• Educate students, residents, other caregivers, and patients; support oncology and transplant-related medication management.
