Job summary

Cleveland Clinic Weston Hospital in Florida is seeking a Clinical Pharmacist to support the inpatient Hematology/Oncology service, including a bone marrow transplant program, with rounds and chemotherapy oversight.

The ideal candidate will have a PharmD or BS in Pharmacy, current RPh licensure, immunization/BLS and ACLS/PALS credentials; experience with Epic and medication safety; join a team known for patient-centered care.

Qualifications

PharmD or Bachelor's of Science in Pharmacy from an accredited College of Pharmacy.
Current state licensure as a Registered Pharmacist (RPh) or eligible within 90 days of hire.
Immunization/BLS certification and completion of a Board of Pharmacy approved immunization course or equivalent within 90 days of hire; ACLS/PALS or ALS certification within 1 year of hire.

Responsibilities

Serve as the primary pharmacist for inpatient Hematology/Oncology service, including bone marrow and stem cell transplant program.
Participate in multidisciplinary rounds; review, verify and optimize chemotherapy and supportive care orders in Epic.
Perform clinical medication monitoring, interventions, safety, and drug information consults; monitor efficacy, toxicity, and adherence to protocols.
Support oncology and transplant-related medication management and assist with operational pharmacy staffing as needed.
