Job summary

HCA Florida North Florida Hospital seeks a Bone Marrow Transplant Clinical Pharmacist to provide comprehensive clinical pharmacy services for the BMT clinic. The role involves anti-neoplastic/ immunomodulating medication management, education, and participation in multidisciplinary rounds and quality projects.

Responsibilities include implementing med therapy with the care team, maintaining medication profiles, patient education on chemotherapy, protocol development, and learner precepting.

Qualifications

• Licensed pharmacist required with ACLS/BCLS certification within 30 days of start.
• Minimum 5 years of experience in relevant clinical pharmacy role.

Responsibilities

• Implement and monitor patient med therapy with the care team.
• Maintain accurate patient medication profiles.
• Educate patients on chemotherapy and supportive regimens.
• Develop protocols and order sets.
• Precept students, residents, and other learners.
• Support BMT/Cellular Therapy with education and transitions of care.
• Conduct immunosuppression monitoring and med reconciliation.
• Review drug interactions and allergies; assess adherence.
• Serve as liaison to improve med-use quality and safety.
