Qualifications

• Bachelor’s Degree in Pharmacy required.
• Licensed Pharmacist with IDFPR.
• Clinical work experience preferred.

Responsibilities

• Direct Patient Care: intervene to improve prescribing and monitoring practices.
• Medication Therapy Management (MTM): provide MTM services and patient education.
• Dosing recommendations based on age, weight, and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams.
• Prepare investigational and standard of care medications for trials.
• Attend rounds and provide pharmacy services in the hematology/oncology setting.
• Oversee chemotherapy preparation and transitions of care.
• Develop policies and protocols to promote safety.
