Qualifications

• Licensed Pharmacist in IL (IDFPR)
• Bachelor’s Degree in Pharmacy required
• Clinical work experience preferred

Responsibilities

• Direct Patient Care: Intervene to improve prescribing, administration, and monitoring practices.
• Medication Therapy Management (MTM): Provide MTM services and patient education as defined by state law.
• Dosing Recommendations: Determine optimal doses based on age, weight, and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams (inpatient and outpatient).
• Prepare investigational and standard chemotherapy; ensure safe preparation and handling.
• Attend rounds and provide pharmacy services in interdisciplinary care at Carle Foundation Hospital.
• Collaborate with providers to improve patient-centered care.
• Monitor adverse effects and therapy plans; ensure adherence and manage toxicities.
• Oversee preparation of chemotherapy; participate in transitions of care and trials.
• Participate in policies, procedures, and safety initiatives for oncology patients.
