Qualifications

• Licensed Pharmacist - IDFPR (Illinois).
• Bachelor’s Degree: Pharmacy (Required).
• Work Experience: Clinical.

Responsibilities

• Direct Patient Care: Intervene to improve prescribing, administration, and monitoring practices.
• Medication Therapy Management (MTM): Provide MTM services and patient education per regulatory act.
• Dosing Recommendations: Determine optimal doses based on age, weight and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams for inpatient/outpatient care.
• Prepare investigational and standard chemotherapy medications for trials.
• Attend rounds and provide pharmacy services in interdisciplinary practice.
• Collaborate with providers to enhance patient-centered care.
• Ensure safe and effective drug therapy management and monitor adverse effects.
• Oversee preparation of chemotherapy and manage transitions of care.
• Participate in policy development and safety initiatives for oncology patients.
