Qualifications

• Licensed Pharmacist (IDFPR) required.
• Bachelor’s Degree: Pharmacy required.
• Clinical work experience preferred.

Responsibilities

• Direct Patient Care: Intervene to improve prescribing, administration, and monitoring.
• Medication Therapy Management (MTM): Provide MTM services and patient education per protocol.
• Dosing Recommendations: Determine doses based on age, weight, and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy for inpatient/outpatient care.
• Prepare investigational and standard medications for clinical trials safely and timely.
• Create and maintain study binders with summaries, protocols, and dispensing procedures.
• Attend hematology/oncology rounds and deliver pharmacy services at Carle Foundation Hospital.
• Collaborate to enhance patient-centered care and safe drug therapy management.
• Monitor adverse effects and regimen-specific adherence and toxicities.
• Oversee preparation of chemotherapy and transitions of care; support trial protocols and staff development.
• Participate in treatment plan development and hospital committees; implement policies for oncology safety.
• Engage in pharmacokinetics dosing service activities and other duties as assigned.
