Responsibilities

• Direct Patient Care: Intervene to improve prescribing, administration, and monitoring practices within the facility.
• Medication Therapy Management (MTM): Provide MTM services and patient education as defined in the Illinois Pharmacy Practice Act.
• Dosing Recommendations: Determine optimal medication doses based on patient age, weight, and renal/hepatic function.
• Collaborate with hematology/oncology and inpatient pharmacy teams for inpatient and outpatient responsibilities.
• Prepare investigational and standard of care medications for clinical trials.
• Attend rounds and provide pharmacy services in the interdisciplinary practice at Carle Foundation Hospital.
• Collaborate with hematology/oncology providers to enhance patient-centered care.
• Provide safe and effective drug therapy management to patients.
• Review, monitor and manage adverse effects.
• Monitor regimen adherence and toxicities for oral chemotherapy.
• Oversee preparation of chemotherapy.
• Assist with transitions of care, staff development, and precept hematology/oncology students.
• Participate in treatment plan development and committees for implementation.
• Develop medication policies, procedures, and safety initiatives.
• Participate in pharmacokinetics dosing service activities and hospital committees.
