Job summary

The ideal candidate is a registered nurse with 1+ year of nursing experience, outpatient experience preferred, and holds or will obtain an RN license in Tennessee; BLS required and OCN within two

Qualifications

• Graduate of an accredited nursing program (required).
• At least 1 year of nursing experience (required).
• Bachelor’s degree in nursing (preferred).
• Outpatient nursing and scheduling experience (preferred).

Responsibilities

• Assess, plan, facilitate, and coordinate care to meet patients’ physical, emotional, and social needs.
• Advocate for timely, high-quality care by guiding patients through the healthcare system.
• Coordinate appointments, referrals, and services to ensure continuous care.
• Provide education on treatment plans, symptom management, and health maintenance.
• Maintain accurate documentation and tracking of patient progress and clinical outcomes.
• Reduce barriers to care through anticipatory guidance, insurance coordination, and community resource navigation.
• Collaborate with oncologists, specialists, and support teams to enhance patient experience and outcomes.
• Educate patients on individualized care plans to reduce unnecessary utilization and improve quality of life.
• Perform RN scope duties, including telephone triage and clinical assessments.
• Participate in clinic activities and assist in care coordination and planning.
• Perform other duties as assigned.
