Job summary

Covenant Health Cancer Center, East Tennessee’s largest nonprofit health system, seeks a Nurse Navigator to provide clinical support, treatment access, education, and coordination of patient-centered cancer care for patients and families.

The Nurse Navigator will collaborate with multidisciplinary teams, educate patients about diagnosis and treatment options, and facilitate access to clinical trials and supportive services.

Qualifications

• Minimum three years oncology experience.
• Valid Tennessee RN license; oncology certification preferred within three years of employment.
• Oncology certification preferred at time of hire or must be pursuing the OCN within three years of employment.

Responsibilities

• Act as the essential link between patients and care providers to access clinical and supportive services.
• Educate patients on diagnosis, treatment options, and clinical trials or fertility preservation.
• Educate patients about disease, treatment side effects, and assist informed decision-making.
• Advocate for patients and coordinate care across multidisciplinary teams.
• Participate in multidisciplinary cancer conferences and follow patients across the care continuum.
• Coordinate patient appointments with labs, imaging, and specialty physicians.
• Identify barriers to care and facilitate referrals to support services.
• Prepare Survivorship Care Plans for eligible patients post-treatment.
