Job summary

Qualifications

• Must be licensed as a Registered Nurse in Montana.
• Three or more years of oncology experience preferred.
• Case management or discharge planning experience preferred.
• OCN certification is preferred.

Responsibilities

• Case finding and patient/family needs assessment.
• Identify and mobilize resources; arrange follow-up with care team.
• Coordinate communication among patient, family, and health care team to ensure continuity of care.
