Job summary

Mercy South Sindelar Cancer Center is seeking an experienced Registered Nurse (RN) Nurse Navigator to guide oncology patients through their cancer journey. You will coordinate care from diagnosis to survivorship, educate families on treatment plans, and collaborate with multidisciplinary teams to ensure timely, high-quality care.

The role requires RN licensure, 2–3 years of nursing experience, and BLS; BSN is preferred, with OCN/ACLS advantageous.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in the state of practice.
• 2–3 years of nursing experience.
• BLS certification required.
• Oncology, infusion, hematology/oncology, care coordination, case management, or nurse navigator experience.
• OCN certification or willingness to obtain.
• Chemotherapy/biotherapy experience.
• ACLS certification.

Responsibilities

• Coordinate and navigate care for oncology patients from diagnosis through survivorship.
• Educate patients and families on diagnoses, treatment plans, meds, procedures, and side effects.
• Facilitate communication among patients, providers, infusion, imaging, and support teams.
• Collaborate with physicians and teams to develop individualized care plans.
• Assist with referrals, appointment coordination, scheduling, and transitions of care.
• Monitor progress and address barriers to care proactively.
• Provide emotional support and connect to community resources and financial help.
• Maintain accurate documentation in EPIC and clinical systems.
• Track outcomes and quality metrics to support evidence-based practice.
• Serve as a clinical resource throughout the cancer care continuum.
