Zusammenfassung

Mercy South Sindelar Cancer Center is seeking a compassionate Registered Nurse (RN) Nurse Navigator to support oncology patients across their cancer journey, coordinating care with oncologists, infusion services, social workers, and other team members.

You will educate patients and families about diagnoses, treatments, and available resources, while helping remove barriers to care and ensuring a high-quality patient experience throughout diagnosis, treatment, survivorship, and follow-up care.

Qualifikationen

• Graduate of an accredited school of nursing.
• Current RN license in the state of practice.
• 2–3 years of nursing experience.
• BLS certification.
• Preferred BSN.
• Oncology, infusion, or nurse navigator experience is a plus.

Aufgaben

• Coordinate and navigate care for oncology patients across diagnosis, treatment, and survivorship.
• Educate patients and families about diagnoses, treatment plans, medications, and resources.
• Facilitate communication among patients, providers, and care teams.
• Collaborate with physicians and interdisciplinary teams to develop care plans.
• Assist with referrals, appointments, scheduling, and transitions of care.
• Monitor progress and address barriers to care.
• Provide emotional support and connect patients with resources.
• Document accurately in EPIC and other clinical systems.
• Track outcomes and support program development.
• Serve as a clinical resource for patients, families, and staff.
