Job summary

Mercy South Sindelar Cancer Center is seeking a Registered Nurse (RN) Nurse Navigator to guide oncology patients and families through their cancer journey, coordinating care from diagnosis to survivorship. You will educate patients, address barriers to treatment, and collaborate with oncologists and the interdisciplinary team.

You will manage referrals, scheduling, and transitions of care while ensuring timely, high-quality care and an exceptional patient experience across all stages of cancer

Qualifications

• Graduate of an accredited school of nursing
• Current RN license in the state of practice
• 2-3 years of nursing experience
• BLS certification
• Oncology, infusion, hematology/oncology, care coordination, case management, or nurse navigator experience
• OCN certification or willingness to obtain
• Chemotherapy/biotherapy experience
• ACLS certification

Responsibilities

• Coordinate and navigate care for oncology patients throughout diagnosis, treatment, recovery, and survivorship.
• Educate patients and families regarding diagnoses, treatment plans, medications, procedures, side effect management, and available support resources.
• Facilitate communication between patients, providers, infusion services, imaging departments, specialty providers, and support teams.
• Collaborate with physicians and interdisciplinary teams to develop and execute individualized care plans.
• Assist with referrals, appointment coordination, treatment scheduling, and transitions of care.
• Monitor patient progress and proactively identify and address barriers to care.
• Provide emotional support and connect patients and families with appropriate community resources, financial assistance programs, and support services.
• Maintain accurate and timely documentation in EPIC and other clinical systems.
• Track patient outcomes and quality metrics to support evidence-based practice and program development.
• Serve as a clinical resource for patients, families, providers, and staff throughout the continuum of cancer care.
