Mercy South Sindelar Cancer Center is seeking a Registered Nurse (RN) Nurse Navigator to guide oncology patients and families through their cancer journey, coordinating care from diagnosis through survivorship. You will educate, support, and collaborate with an interdisciplinary team to ensure timely, high-quality care.

This Full-Time position requires BSN preferred, BLS/ACLS, and 2–3 years nursing experience, with oncology/infusion exposure ideal.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in the state of practice.
• 2–3 years of nursing experience.
• BLS certification.
• BSN preferred.
• Oncology, infusion, hematology/oncology, care coordination, case management, or nurse navigator experience.
• OCN certification or willingness to obtain.
• Chemotherapy/biotherapy experience.
• ACLS certification.

Responsabilités

• Coordinate and navigate care for oncology patients throughout diagnosis, treatment, recovery, and survivorship.
• Educate patients and families regarding diagnoses, treatment plans, medications, procedures, side effect management, and available support resources.
• Facilitate communication between patients, providers, infusion services, imaging departments, specialty providers, and support teams.
• Collaborate with physicians and interdisciplinary teams to develop and execute individualized care plans.
• Assist with referrals, appointment coordination, treatment scheduling, and transitions of care.
• Monitor patient progress and proactively identify and address barriers to care.
• Provide emotional support and connect patients and families with community resources and support services.
• Maintain accurate and timely documentation in EPIC and other clinical systems.
• Track patient outcomes and quality metrics to support evidence-based practice and program development.
• Serve as a clinical resource for patients, families, providers, and staff throughout cancer care.
