Job summary

Mercy South Sindelar Cancer Center is seeking a Registered Nurse (RN) Nurse Navigator to guide oncology patients and families through the cancer journey, coordinating care, education, and access to resources. Collaboration with oncologists, infusion, imaging, and other teams ensures timely, high-quality care and an exceptional patient experience.

Qualifications

• Graduate of an accredited school of nursing.
• Current Registered Nurse (RN) license in the state of practice.
• 2-3 years of nursing experience.
• Basic Life Support (BLS) certification.

Responsibilities

• Coordinate and navigate care for oncology patients throughout diagnosis, treatment, recovery, and survivorship.
• Educate patients and families regarding diagnoses, treatment plans, medications, procedures, side effect management, and available support resources.
• Facilitate communication between patients, providers, infusion services, imaging departments, specialty providers, and support teams.
• Collaborate with physicians and interdisciplinary teams to develop and execute individualized care plans.
• Assist with referrals, appointment coordination, treatment scheduling, and transitions of care.
• Monitor patient progress and proactively identify and address barriers to care.
• Provide emotional support and connect patients and families with community resources and support services.
• Maintain accurate and timely documentation in EPIC and other clinical systems.
• Track patient outcomes and quality metrics to support evidence-based practice and program development.
• Serve as a clinical resource for patients, families, providers, and staff throughout the continuum of cancer care.
