Job summary

HCA Healthcare is seeking an Adult Oncology Nurse Navigator to support oncology patients at CJW Medical Center in the Richmond, VA area. The role is onsite with a Monday–Friday schedule and focuses on coordinating care, educating patients, and guiding families through the healthcare system to improve outcomes.

You will collaborate with a multidisciplinary team, manage care pathways, and assist with access to services and community resources.

Qualifications

• 1-3 years of experience in Nursing – Required
• 3+ years of experience as a Nurse Navigator and/or patient educator in service line – Preferred

Responsibilities

• Serves as a patient advocate.
• Assesses patient’s medical and psychosocial needs related to navigation.
• Identifies barriers to care and connects to resolutions.
• Identifies health disparities and helps remove them.
• Provides education, outreach, and support to patients and families.
• Guides patients and families through the healthcare system and plan of care.
• Coordinates appointments and schedules to streamline care pathways.
• Attends Multidisciplinary Meetings and care planning conferences.
