Qualifications

• Current RN license in the state where services will be provided or valid multi-state license via eNLC.
• Basic Life Support (BLS) certification within 30 days of hire.
• OCN or BMTCN certification required within 3 years of hire.
• Three years of nursing experience.

Responsibilities

• Assures delivery of comprehensive cancer services through coordination of expert patient care for adult hematology/oncology patients.
• Coordinates and monitors all phases of cancer services including prevention, diagnosis, treatment, survivorship, and end-of-life care.
• Acts as a liaison between patients, physicians and hospital departments to ensure seamless care.
• Collaborates with financial counselors and social services to complete financial assessments.
• Guides and participates in care planning, referrals, and problem-solving to optimize patient functioning.
