Trinity Health IHA Medical Group in Michigan seeks a Care Manager - Oncology to join our outpatient care team. You will lead care coordination, clinical triage, patient education, and ongoing support to improve outcomes and patient experience.

The role offers a hybrid schedule (up to 2 days per week after orientation). An RN program graduate with Michigan RN license and CPR is required; CCM/CCP certification is preferred.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Valid, unrestricted RN license in the State of Michigan; CPR certification.
• 3–5 years of oncology clinical experience in ambulatory or inpatient settings.

Responsabilités

• Provide care management and coordination for patients beginning chemotherapy.
• Assess and triage patient needs by phone and in-person visits.
• Deliver clinical guidance and patient education across transitions of care.
• Support post-discharge follow-up and continuity of care.
• Collaborate with providers, pharmacists, and behavioral health specialists.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and remove barriers to care.
