Responsibilities

• Assess patient health data and determine care needs within 24 hours or policy window.
• Conduct comprehensive assessments across physical, psychological, spiritual, and educational domains.
• Document findings clearly and notify physician of status changes.
• Identify outcomes and plan care to meet patient and family needs.
• Coordinate with care teams and discharge planning as condition evolves.
• Educate patients and families about illness, treatment, meds, and aftercare.
