Qualifications

Responsibilities

• Evaluates patient progress toward outcomes and revises plans as needed.
• Performs comprehensive patient assessment and collects health data.
• Communicates care plan to patient/family and interdisciplinary team.
• Provides patient and family teaching tailored to learning needs.
• Identifies community resources to support patient self-management.
• Supports professional development of peers and participates in research and evidence-based care.
