Job summary

Luminate is seeking a Home Infusion Nursing Coordinator in Maine to support the Lotus Home Infusion program, coordinating with oncology teams to drive adoption and deliver educational materials. You will conduct one-on-one meetings with physicians and nursing staff, identifying friction points in home infusion workflows and delivering targeted improvements.

Qualifications

• Graduate of an accredited school of nursing; BSN preferred.
• Current state license in Maine as a Registered Nurse in good standing.
• Oncology nursing experience preferred.
• ONS Provider Card within 12 weeks of hire (OCN desirable).

Responsibilities

• Screen patients for suitability for home infusion in coordination with their physician and multi-disciplinary care team.
• Complete and support others to complete patient screening, eligibility assessments, onboarding, scheduling visits in the EMR, and ensuring all steps are completed before drugs are picked up by the home infusion nurse.
• Ensure post-infusion communication and EMR documentation steps are completed in a timely manner.
• Collaborate with internal clinical, operations, and commercial teams to deliver Lotus home infusion program implementation and ongoing operational excellence.
• Monitor the patient onboarding pipeline and address bottlenecks to maximize capacity.
• Identify and elevate implementation risks, barriers, and opportunities to support program delivery.
• Deliver training and ongoing support to oncology teams for confident adoption of Lotus home infusion workflows.
• Drive continuous improvement to enhance processes, stakeholder experience, and program performance.
• Provide regular dashboard review of home infusion performance at monthly Steering meetings.
