Job summary

Luminate is seeking Oncology Home Infusion Nurses (RN) to join our growing US Healthcare Services team. You will coordinate and perform infusion nursing care for patients in the home setting, administer skilled nursing care, teach patients and caregivers, and collaborate with the National Director of Nursing to establish care priorities.

Graduate from an accredited nursing program; RN license; CPR; 2+ years infusion therapy experience; ONS Provider Card; OCN desirable.

Qualifications

• Graduate of an accredited nursing program required.
• Current RN license in good standing.
• CPR certification required; OCN desirable.

Responsibilities

• Abides by policies and standards of practice; follows guidelines and laws.
• Provides infusion and skilled nursing care, patient assessments, teaching, planning and documentation.
• Pre-discharge patient assessments in hospital or clinic when appropriate.
• Maintains accurate patient care documentation and reports.
• Communicates patient status and changes to supervisors and prescribers.
• Provides 24-hour availability as required.
• Represents self professionally and ethically at all times.
• Participates in quality assurance activities and team meetings.
