Qualifications

• Completion of a PGY2 Oncology Residency or PGY1 Ambulatory Care Residency with 3+ years in oncology.
• 3+ years as a clinical pharmacist in oncology with 1 year under a Collaborative Practice Agreement.

Responsibilities

• Provide comprehensive medication management including reconciliation and dose optimization.
• Collaborate with care teams to order individualized care plans under a Collaborative Practice Agreement.
• Monitor therapy effectiveness and safety, identifying optimization needs and intervening as necessary.
• Support quality improvement and REMS initiatives.
