Job summary

Northwestern Medicine seeks a Clinical Pharmacist to ensure safe, effective medication use and to support patient-centered care across the hospital. The clinician will identify and resolve medication problems, provide expert drug information to clinicians and patients, and maintain accurate records within the EMR and pharmacy systems.

The role requires an Illinois license and a Doctor of Pharmacy or PharmD, with responsibilities spanning verification, dispensing, and collaboration with nursing

Qualifications

• Bachelor’s Degree in Pharmacy or Doctor of Pharmacy.
• Active Clinical Pharmacist license in the state of Illinois (IDFPR).

Responsibilities

• Identify, prevent, and resolve medication-related problems for patients.
• Provide authoritative information to health care professionals, patients, and the public.
• Ensure drug distribution components function as an integrated system for patient-centered care.
• Maintain documentation in the EMR and Therapy Management System and oversee pharmacy operations.
