Job summary

Trinity Health IHA Medical Group seeks a compassionate Care Manager - Oncology to join our outpatient care team in Michigan. This hybrid role includes two days remote work and involves clinical triage, care coordination, patient education, and ongoing support to improve patient outcomes and experiences.

With 3–5 years of oncology experience as a registered nurse, you will collaborate with providers and other health professionals, manage referrals and prior authorizations, and support transitions

Qualifications

• RN with unrestricted Michigan license.
• CPR certification.
• CCM/ CCP certification preferred.

Responsibilities

• Act as an integral member of the outpatient care team.
• Provide care management and navigation for patients beginning chemotherapy.
• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance and patient education on treatment plans.
