Job summary

Trinity Health IHA Medical Group in Michigan is seeking a compassionate Care Manager - Oncology to join our outpatient care team. The role focuses on clinical triage, care coordination, patient education, and ongoing support to improve outcomes and the patient experience.

You will collaborate with providers, coordinate transitions of care, and assist with medication refills and prior authorizations while supporting chronic disease management for oncology patients.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Active, unrestricted RN license in Michigan.
• CPR certification.
• CCM/CCP certification preferred.
• 3–5 years oncology clinical experience in ambulatory/acute care settings.

Responsibilities

• Provide care management and care coordination for oncology patients throughout the care journey.
• Triage patient needs via phone and in-office visits and provide clinical guidance.
• Coordinate with providers, pharmacists, and other care team members to support transitions of care.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Assist patients in accessing community resources and removing barriers to care.
• Serve as a clinical resource and leader within the care team.
