Job summary

Trinity Health IHA Medical Group is seeking a compassionate Care Manager - Oncology to join our outpatient care team. You will coordinate care for oncology patients, provide clinical triage, patient education, and support through transitions of care.

As an RN with 3–5 years oncology experience in outpatient, home health, or hospital settings, you will collaborate with providers, pharmacists, and behavioral health specialists to improve outcomes while maintaining an efficient schedule.

Qualifications

• RN degree from an accredited program.
• Unrestricted RN license in Michigan; CPR certification.
• CCM/CCP certifications preferred.

Responsibilities

• Act as a core member of the office-based care team.
• Provide care management, coordination and navigation for patients starting chemotherapy.
• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, patient education, and care coordination.
