Job summary

Trinity Health IHA Medical Group in Michigan seeks a compassionate Care Manager - Oncology to join our outpatient team. You will guide patients through their cancer care journey with clinical triage, care coordination, patient education, and ongoing support to improve outcomes.

As an integral member of the office-based care team, you will coordinate with providers, pharmacists, behavioral health specialists, and caregivers, manage referrals and prior authorizations, and help patients navigate

Qualifications

• Graduate of an accredited Registered Nurse program.
• Valid, unrestricted RN license in Michigan with CPR certification.
• CCM/CCP certification preferred.
• 3–5 years of oncology clinical experience in ambulatory care, home health, SNF, or hospital in the past five years.

Responsibilities

• Provide care management, care coordination and navigation for oncology patients.
• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, patient education, and care coordination.
• Support patients through transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, behavioral health specialists, and other healthcare partners.
