Job summary

Trinity Health IHA Medical Group in Michigan is seeking a compassionate Care Manager - Oncology to join our outpatient care team. You will guide patients through chemotherapy journeys with clinical triage, care coordination, and education to improve outcomes and patient experience.

As an RN with 3–5 years of oncology experience, you will assess needs by phone and in person, coordinate care across providers, manage referrals and authorizations, and support transitions of care while promoting

Qualifications

• Graduate of an accredited RN program.
• Knowledge of oncology care and chronic disease management desirable.
• CPR certification required.

Responsibilities

• Active member of the outpatient care team.
• Provide care management, care coordination and navigation for chemotherapy patients.
• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance and patient education.
• Support transitions of care and post-discharge follow-up.
• Collaborate with providers, pharmacists, and other health professionals.
• Manage referrals and prior authorizations.
