Job summary

The University of Kansas Hospital in Kansas City area is seeking a Nurse Navigator for the Cancer Center to coordinate patient care and promote timely access to treatment. You will act as a clinical expert across care settings, guiding patients and families through intake, planning, and ongoing care while collaborating with a multidisciplinary team.

Required RN licensure, a Bachelor’s in Nursing, and 3+ years of nursing with 1+ year in case management.

Qualifications

Bachelor’s degree in Nursing required.
3+ years of nursing experience.
1+ years of case management or ambulatory care coordination experience.
Active RN license in the state of residence.
BLS certification.

Responsibilities

Assess needs for new patients referred to a specialist prior to their first visit.
Document patient assessments and interventions in the EMR.
Refer patients to the program’s supportive services as needed.
Provide educational and emotional support during intake and treatment planning.
Coordinate scheduling with multidisciplinary providers and locations.
Assist in coordinating diagnostic studies prior to first visit when possible.
Develop relationships with referring physicians and ensure follow-up communication.
Support and participate in quality improvement initiatives.
