Job summary

Trinity Health IHA Medical Group is seeking a Nurse Care Manager focused on Oncology to join our outpatient care team. The role emphasizes care coordination, patient education, and clinical triage to improve outcomes and patient experience.

The position is a hybrid schedule with two remote days per week, day shift, and opportunities for professional growth within a compassionate, patient-centered culture in Southeast Michigan.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Active, unrestricted RN license in Michigan with CPR certification; CCM/CCP preferred.
• 3–5 years of oncology experience in primary/ambulatory care, home health, SNF, or hospital within the past five years.

Responsibilities

• Provide care management and navigation for patients beginning chemotherapy.
• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance and patient education; support transitions of care.
• Collaborate with providers, pharmacists, behavioral health specialists, and other healthcare partners.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.
