Job summary

Orlando Health Cancer Institute is seeking a Care Coordination RN to collaborate with clinical teams to assess patients at risk and optimize discharge planning across inpatient, emergency, and ambulatory settings.

You will work with Patient and Family Counselors, Utilization Review Nurses, and IT tools to improve care outcomes, ensure timely access to medications and DME, and support patients through the healthcare system.

Qualifications

• Bachelor of Science in Nursing (BSN) required for new hires/transfers.
• Minimum of 3 years direct clinical experience in the target population.
• Current RN license in Florida and current BLS/Healthcare Provider certification.

Responsibilities

• Lead care coordination across inpatient, emergency, and outpatient settings.
• Collaborate with social workers and utilization nurses to plan discharge.
• Monitor discharge plans daily and adjust for patient condition.
• Educate patients and families on self-management and care options.
• Coordinate access to prescriptions, DME, and services.
• Report outcomes and track progress toward care plan goals.
• Ensure patient safety and compliance with policies.
