Job summary

Orlando Health Cancer Institute is seeking a highly skilled RN for care coordination across inpatient, emergency, and ambulatory care settings. The role emphasizes collaborating with social workers and nurses to facilitate timely discharge planning and patient advocacy within a multidisciplinary cancer care team.

You will educate patients and families on care options and coordinate services to support positive health outcomes, leveraging IT tools for reporting and population health initiatives.

Qualifications

• Bachelor of Science in Nursing (BSN) required.
• Three years of direct clinical experience in the target population.
• Current Florida RN license and BLS certification required.

Responsibilities

• Lead continuity of care across inpatient, emergency, and ambulatory care settings to ensure integrated delivery.
• Collaborate with Patient and Family Counselors/Social Workers and Utilization Review Nurses to support discharge planning.
• Monitor progress toward discharge plans and adjust as patient condition or family needs change.
• Educate patients and families on evidence-based standards of care and self-management strategies.
• Coordinate with IT tools to support registry reporting and population health outcomes.
