Job summary

Qualifications

• Minimum two years clinical RN experience in acute or ambulatory care.
• Two years Oncology clinical experience.

Responsibilities

• Evaluates member needs with PCPs and specialists to develop case management plans.
• Monitors effectiveness of plans and updates as needed.
• Coordinates care across providers, including transfers and authorizations.
• Serves as clinical liaison with outside agencies and employers.
• Prepares reports, communicates changes, and develops protocols.
• Advocates and educates patients to empower self-management.
• Develops patient/family education on self-management.
• Facilitates referrals for services and continued care.
• Ensures proper documentation and data collection for outcomes.
