Job summary

Kaiser Permanente is seeking an RN for Per Diem outpatient palliative care. The role involves coordinating with physicians, staff, and external providers to develop and implement a comprehensive, multi-disciplinary treatment plan across the care continuum.

You will monitor care, make evidence-based recommendations, educate patients and families, arrange follow-ups, and ensure compliance with regulatory standards. On-site collaboration with the care team is essential.

Qualifications

• 1 year acute care RN experience.
• 2 years home health or hospice RN experience.
• California RN license and BLS certification

Responsibilities

• Plans, develops, assesses and evaluates care provided to members.
• Recommends alternative levels of care and ensures regulatory compliance.
• Develops patient education plans focused on self-management.
• Coordinates care/services with utilization and quality reviewers.
• Arranges and monitors follow-up appointments.
• Interprets regulations, plan benefits and procedures for members, providers and agencies.
