Job summary

Kaiser Permanente seeks an experienced RN to coordinate care across physicians, staff, and providers for defined patient populations. You will plan and implement a comprehensive, multidisciplinary approach to manage health conditions, resources, and protocols.

Collaborate with the team to develop treatment plans and monitor care across the continuum. Responsibilities include evaluating patient care plans, coordinating with care teams, and ensuring compliance with regulatory requirements.

Qualifications

• Active RN license in California.
• 1+ year of clinical RN experience; 2+ years in home health or hospice preferred.
• Basic Life Support certification required.
• Case management certification preferred.

Responsibilities

• Plans, develops, assesses and evaluates care provided to members.
• Evaluates and develops baseline medical and psychosocial evaluations and individualized patient care plans.
• Recommends alternative levels of care and ensures compliance with regulations.
• Develops patient/family education plans focused on self-management.
