Job summary

Qualifications

• Minimum two years of clinical RN experience in acute or ambulatory settings.
• Two years of Oncology clinical experience is preferred/required.
• California RN license and BLS certification required.
• OCN or cancer program certification is preferred within one year of hire.

Responsibilities

• Collaborate with physicians to develop and implement individualized care plans.
• Coordinate access to health services across multiple providers and disciplines.
• Monitor care outcomes and modify care plans as needed to ensure quality and cost-effectiveness.
• Act as a clinical liaison with outside providers, facilities, and agencies as required.
• Educate patients and families to promote self-management and adherence to care plans.
